OKLAHOMA CITY -- The death of Bob Mills' 92-year-old mother prompted the Health Department to investigate The Health Center at Concordia.

Lorean Gregory was staying at the center. Mills said someone propped open a door that should have been shut. No one was watching as his wheelchair-bound mother made her way down the hallway and fell down a flight of stairs.

Gregory was hospitalized due to injuries sustained in the fall. While in the hospital, her health continued to declined and she died two months later.

"Had there been proper staff and adequate monitoring, somebody would have found her and we wouldn't be having this conversation," Mills said.

The report mentioned another resident who managed to leave the nursing home but was quickly caught and returned.

The director of nursing said they've now "increased supervision" and "door alarms have been placed to make a safer environment."

The state cited Concordia for six different problems, including not sending Gregory's medical history with her when she went to the hospital which resulting in her getting stitches with little anesthetic and no pain medication.

Concordia said it disagrees with some of the findings.

State investigators plan to return to Concordia in the following months to follow up.
