A police report outlines what witnesses saw at the new Bank of Oklahoma tower under construction near Hudson and Shartel on Friday.

Based on the details released Tuesday, it was an unimaginable accident.

The construction crews were in agony that day after what happened to 33-year-old Alex Baiza while working.

One crew member standing next to him said Baiza was using a scissor lift on framework and had just successfully moved one part.

When Baiza tried to go up for more he "must have thought he had changed the mode from forward and back to up and down."

But witnesses said it was left in forward and back mode. So when he attempted to go up, it went forward instead.

Then "the front of the lift drove off the side of the building and began to tip over the side," the witness said.

Baiza "was able to grab onto some wiring" to pull himself from the falling lift, according to the report.

But because he was tied into the machine, he was pulled down.

The witness said he tried to reach out for Baiza, but the cable pulled him down faster than he could reach out.

The Occupational Safety and Health Administration is investigating.

News 9 reached out to their office Tuesday, but have yet to hear back.
