A new survey released Tuesday ranked the health of 3,000 U.S. counties based on exercise, smoking rates, clinical care and more.

The Robert Wood Johnson Foundation released its annual County Health Rankings for 2012.

In Oklahoma, the healthiest county is Cleveland County in central Oklahoma.  It was ranked number 1 of the state's 77 counties.

Tulsa County was ranked 24th and coming in last place at 77th was Seminole County.

The survey looks at health factors such as clinical care, health behaviors, social and economic factors and physical environment.  It also shows health outcomes for each Oklahoma county including mortality and morbidity rankings.

This year the survey added a couple of new health-related measures including how many fast food restaurants are in each county and the levels of physical inactivity for residents of each county.
